ohsam comment fic meme: Without a hospital nearby, Dean is forced to carry out some emergency surgery on Sam (ie a chest tube or something equally as serious). Maybe Dean calls someone for advice and they guide him through it step by step or maybe he has to do some super fast research and he's all on his own.
Gen with plenty of h/c, freaked out Dean and maybe even some supportive Sam telling Dean everything will be OK and that he trusts his brother.
